Business update (quarterly tax newsletter)

Read the Business Update, a quarterly newsletter for businesses to address unemployment-insurance tax issues. The editions include information about recent legislation, electronic filing, tax rates, reporting requirements, and other important information that will help you stay current.



Forms

Business change (Form 5208C-1) – Use this form to report changes to your business status, including change of address. You must include your name and phone number for the change to be processed. (PDF, 170KB)

Update ownership information (Form 5208C-2) - Required for all changes in owners or officers. (PDF, 351KB)

Amended tax and wage report (Form 5208D) – Use this form to make a change to your tax or wage report from the previous quarter. (PDF, 138KB)

Exempting corporate officers – Use this link to select the proper form for your corporation to exempt corporate officers from unemployment insurance.

Claim for refund (Form EMS 5227) – Use this form to request a refund of previously paid contributions, interest or penalties. (PDF, 26KB)

Request for unemployment insurance tax payment method (Form EMSX 5200-99) – This form is used by non-profit organizations or political subdivisions to determine a tax payment method. (PDF, 25KB)

Voluntary election (Form EMS 5203) – Submit this form if you would prefer to report and pay taxes but are not required to. If approved, your employees will be eligible to receive unemployment benefits if they are laid off. (PDF, 37KB)

Report new client information/report of termination – Required whenever a Professional Employer Organization adds or drops a client (see submitting instructions). (PDF, 320KB)
