Forms

Business change form (PDF, 170KB) – Use this form to report changes to your business status, including change of address. You must include your name and phone number for the change to be processed.

Update ownership information (PDF, 351KB) - Required for all changes in owners or officers.

Amended tax and wage report (PDF, 138KB) – Use this form to make a change to your tax or wage report from the previous quarter.

Claim for refund (PDF, 26KB) – Use this form to request a refund of previously paid contributions, interest or penalties.

Request for unemployment insurance tax payment method (PDF, 25KB) – This form is used by non-profit organizations or political subdivisions to determine a tax payment method.

Voluntary election (PDF, 94KB) – Submit this form if you would prefer to report and pay taxes but are not required to. If approved, your employees will be eligible to receive unemployment benefits if they are laid off.

Report new client information/report of termination (PDF, 320KB) – Required whenever a Professional Employer Organization adds or drops a client (see submitting instructions).

PEO power of attorney (PDF, 801KB) – Required for Professional Employer Organizations to represent client.
